2) Passport Acknowledgement slip
3) Copy of online application form (signed and dated)
4) One passport photograph
5) Expired Nigerian Passport (if not presented will be considered lost)
6) One copy of the expired Nigerian Passport data page
7) Self-addressed return USPS envelope only from the post office with
overnight
Additional information for first time applicants:
8) Birth Certificate
9) Copy of both parents Nigerian Passport
10) Letter of consent to be signed and notarized by both parents.
